Stage 1 Hypertension

Real-time escalation
Every reading is triaged in real time. High readings trigger immediate clinician callbacks with clear protocols.
Level 1: Reassurance
ImmediateTrigger: Normal reading (SBP <140, DBP <90)
Action: Automated message: 'Your reading is in a safe range. Keep taking your meds.'
Level 2: Symptom Check
ImmediateTrigger: Elevated reading (SBP 140–159)
Action: System asks: 'Are you having chest pain, shortness of breath, or severe headache?'
Level 3: Clinician Callback
Within 1 hourTrigger: High reading (SBP 160–179) with symptoms OR repeat high reading
Action: Licensed nurse calls within 1 hour. Assesses symptoms, medication adherence, and next steps.
Level 4: Urgent Callback
Within 5–15 minutesTrigger: Very high reading (SBP ≥180) OR symptomatic crisis
Action: Immediate clinician callback. May recommend telehealth, urgent care, or 911.
Level 5: Emergency
ImmediateTrigger: Hypertensive emergency (SBP ≥180 + severe symptoms) OR member reports emergency symptoms
Action: Immediate guidance to call 911 or go to nearest ED.
